The fax must include:

• An official Purchase Order Number from your university or Credit Card Details (Number, Expiration Date, Name on Card, Billing Address).
• Your university's invoice address (where we send the invoice to get payment)
• Your university's delivery address
• Your name , phone number, and email address so we can contact you if we have any questions
